Combined Excise Tax
Monthly Quarterly Annual Multi-Purpose	This form is used for reporting your business income, sales tax, and use tax. Use this form if you do not qualify to use the tax returns below.
Business & Occupation Activities
Monthly Quarterly Annual	This form is customized for taxpayers who report under certain B&O tax classifications and do not collect retail sales tax. Taxpayers can also report use tax on this form.
Retailing and Other Activities
Monthly Quarterly Annual	This form is customized for taxpayers who collect retail sales tax and report under certain B&O tax classifications.
Sales Tax Remittance (STR)
Annual	Use this form if: your total annual business income is less than $28,000 you owe retail sales tax under $2,000 you owe no other taxes (The retailing B &O tax is not due if your retail sales tax is less than $2,000.) If this form does not apply to you, use the Combined Excise Tax Return above.
